Medical Release

I hereby give permission for my child(ren) to attend children's programming at Scottsdale Bible Church. In the event of an emergency in which my child is in need of immediate hospitalization, medical attention or surgery, and after reasonable efforts have been made to contact me or my spouse and we cannot be located for the purpose of consenting thereto, consent that emergency attention may be given to any person standing to loco parentis to my child pursuant of A.R.S. 44-133. I understand that my own medical insurance will be the primary coverage. In some limited cases involving an accident on campus, SBC insurance might be available to cover expenses not covered by my insurance, but the SBC coverage is limited to the policy limits of the SBC “Medical Pay” portion of its general liability insurance. It is understand that no medication will be administered to my child and that my child will obey all regulations of and instruction of the leaders.
